Calaveras County Residential Damage Report Form

The damage assessment threshold criteria for the California Office of Emergency Services (CalOES) threshold for damage reporting is each home and/or business or other eligible institution, sustaining uninsured losses of at least 40 percent or more of the estimated fair replacement value or pre-disaster fair market value of the damaged property. Damages must be confirmed through a state inspector during a formal Property Damage Assessment (PDA). The County is currently working with CalOES to determine whether the county-wide threshold for a Small Business Administration disaster declaration has been met. Submittal of this form is for gathering information purposes and does not ensure or guarantee that resources will be available or provided.
